Mid-Year Feedback
The purpose of requesting mid-year feedback is to learn:

• how we can improve your Higher Ground experience and
• how you are progressing with your personal growth and well-being goals

For all questions with ratings, rate from 1 to 7 with 1 as the lowest rate of satisfaction and 7 indicating greatest satisfaction. Please provide comments when prompted.

2. Rate the quality of the cohort gatherings. (1-7)

3. Rate (on a scale of 1-7) the level of your well-being in each of the 5 dimensions at the mid-point of your Higher Ground experience.

8. Rate your level of confidence in achieving your well-being goals.
